Membership Form

The Membership form is available as a PDF which can be completed electronically or printed and then completed by hand. The link below will open either in your browser or in another program/app, depending on the setting in your browser. However not all browsers will enable you to complete the form electronically and you may need another program/app to do this.

Adobe Acrobat Reader is free and can be downloaded from https://acrobat.adobe.com/uk/en/acrobat/pdf-reader.html, from Google Play Store or Apple Store. Other PDF apps are also available.

The boxes in red are mandatory. Please ensure that you sign the form and then send by email or post to the Membership Secretary as stated on the form.

Instructions for completing the form electronically

To complete the form electronically, you can type in the boxes or select items from the drop-down box (subscription amount). Please check that the year is correct.

To sign the form electronically in Adobe Acrobat Reader:

  • click on the 'Fill & Sign' icon Fill Sign. If it shows a page 'Who needs to fill and sign?' then click on the  'Fill and sign' button.
  • Click on the Sign icon above the document Sign, then on 'Add Signature'. The following box will appear Signature box
  • Select 'Draw' and sign on the line.
  • Click 'Apply' and then click in the Signature box in the PDF form to place your signature there. 

Save the file. If you have not signed it electronically, you will need to print the form and sign it.
